Below is a link to the fillable version of the parish registration form. Note: When you initially click on the "download file" button, the document may appear in your web browser in such a way that the fillable fields do not align with the document. If this happens, download the file again from that web-viewer and then open the file from the downloads stored on your computer. When you have filled out the form, click "save as."
